In accordance with Regulatory authorities, your Property and Casualty Insurance Company is giving you this notice to tell you how we may use information about you or your policy. This notice also advises you of your right to keep this information from being shared with other companies that may be affiliates of Fulmont Mutual Insurance Company or other business associates (non-affiliates) under certain circumstances.

Please note that you will receive this notice on an annual basis after the initial mailing. As required by the Regulation, you will receive one privacy notice for each policy that you have in effect with Fulmont Mutual Insurance Company.

• Information we receive from you on applications or other forms, which could include your name, address and social security number;

• Information about your transactions with us, our affiliates or other non-affiliated third parties, which could include information about balances, payment history and parties to the transaction;

• Information we receive from a consumer-reporting agency, which could include information regarding a consumer’s creditworthiness and credit history.

• Information we may obtain from third parties depends on the type of policy you may have with us, but may include claims reports and property inspection reports.

We may share the previously described nonpublic personal financial information that we collect (as permitted by law) with the following:

• Your Insurance Agent or Broker - to provide you with the best possible service, your agent/broker is your direct link to your insurance company.

• A government agency, self-regulatory organization, or other organization pursuant to an examination of our records and/or practices;

• Your attorney, trustee or anyone else who represents you in a fiduciary capacity or has a legal interest in your policy;

• Persons to whom a court order requires us, by subpoena or court order to provide information;

• Persons or Organizations that conduct Actuarial or Research studies, provided that individuals might not be identified in any report.

• Inspection or Loss Prevention Companies - in order to inspect your home to help you determine if your coverage is adequate or if hazards exist;

• Loss Adjusting Companies or Investigators - to provide information to properly represent you or to assist you in reaching a satisfactory settlement for your loss;

• Your Mortgage Company - to assist with information they need to pay your insurance premiums or to be correctly listed on your policy;

• An Insurance Support Organization or another insurer to prevent or prosecute fraud or to underwrite a risk or detect criminal activity;

• Insurance Rate Advisory Organizations;

• Consumer Reporting Agencies;

• Our Attorneys, Accountants or Auditors;

• We may share the same kinds of information as described above about our former customers with the same types of third parties as described above, as well as with affiliates and non-affiliated third parties as permitted by law.

3. Call (518) 762-3171, extension #116. Follow the voice-mail directions to leave your name, address, policy number and telephone number for us to confirm your information if necessary. (For your ease, complete the opt-out notice located on this form prior to making the call.)

Your opt-out form must reach us within 30 days of your receipt of our privacy notice. Your opt out request will take effect on the 7th day following receipt of your request, to allow for notification to all applicable affiliated or non-affiliated third parties.

If you share your account with another person, either of you may opt out of disclosures (other than disclosures permitted by law) for both of you.

We restrict access to non-public personal financial information about you to our employees, agents and subcontractors who need this information to provide products and services to you. We maintain physical, electronic and procedural safeguards that comply with federal & Company regulations to guard your non-public personal financial information.
